Well, I guess, partially yes. As far as I know the system in my country considers the whole of the film. The context, e.g. of an act of violence, is also very important while rating. Nothing is counted per se, e.g. how many profanities or how many punches. Nudity, per se, is never an issue here & sex very seldom; the issue is violence.

'Course it could be the depiction & description of Earl, and then Ennis's imagined view of Jack and the tire iron. Both scenes were pretty graphic and depicted much more horrific actions than Ennis's punch. Oh yeah, then there's the bikers, the black & blue Eagle bar . . . .
